Fora 6 3 in 1 Test Strip used in conjunction with the Fora 6 Connect Multifunctional monitoring system, are intended for the quantitative measurement of blood glucose, hematocrit and hemoglobin in capillary whole blood collected from the finger. It is intended for in vitro diagnostic use only. This system is not designed for the diagnosis or screening of diabetes mellitus.

Characteristics:

• Compatible with Fora 6 Connect equipment.
• Store test strips in a dry and cool place between 2°C and 30°C, and relative humidity below 85%.
• The blood glucose meter displays plasma-equivalent results and is presented in mg/dL or mmol/L. Hematocrit results are indicated in% and hemoglobin results in g/dL.

Usage advice:

Use the test strip immediately after removing it from the vial or individual aluminum packaging. Close the bottle immediately after removing the strip. Do not use test strips after the expiration date. Always keep the bottle closed. Store test strips only in the original vial. Do not transfer them to a new bottle or any other container. Do not touch the test strips with wet hands. Do not bend, cut or alter the test strip. Keep test strips out of direct sunlight. Do not store test strips in high humidity environments.
